Metro Professional Services is currently performing on a contract at Maxwell AFB that will be located at the Maxwell Ambulatory Healthcare Center, also referred to as the “Medical Treatment Facility” (MTF). We will be providing Personal Services for customers at the MTF.
JOB DESCRIPTION:
We will serves as part of the Primary Care Manager (PCM) team within the Air Force Medical Home (AFMH) model of health care delivery, and provide nursing assistance in the full spectrum management of professional and patient care activities within their scope of practice. We are expected to assess and identify individual patient care needs. Apply evidence-based practice (EBP) and nursing processes in making appropriate clinical decisions for pediatric, adolescent, adult, obstetric, and geriatric patients. Ability to synthesize data from a variety of sources and make appropriate clinical decisions.
Along with this, we will coordinate patient care through a continuum and facilitates the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness. Ensuring compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment facility. Finally, we will provide care within ethical and legal boundaries.
EDUCATION:
Nationally Accredited Baccalaureate Degree program in Nursing and holds current certification as a Registered Nurse (RN)
There are further Continuing Medical Education (CME)/Certified Education Units (CEU) that will also need to remain current
Shall have and maintain an active, valid unrestricted, current medical license with no limitations, stipulations or pending adverse actions to practice nursing as a registered nurse in any US State
EXPERIENCE:
Required Experience:
Shall have at least twenty-four (24) months of experience within the last 36 months. This experience shall have been in a primary care clinic setting.
Desired Experience:
Must have knowledge and experience, or demonstrate comprehension during training, with software and databases currently employed at the MTF (e.g. Microsoft Office, Access, Excel and PowerPoint; Composite Health Care System [CHCS], Armed Forces Health Longitudinal Technological Application [AHLTA]). Must possess knowledge, skills and computer program literacy to collect and analyze data.
Must have knowledge and experience in Patient Advocacy, Patient Privacy, and Customer Relations.
Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
Familiarity with Department of Defense (DoD), Federal, State, non-profit healthcare systems and organizations, to include DoD Tri-Service Military Healthcare Systems, TRICARE purchased care system, Medicare, Medicaid and the Veterans Administration (VA).
Health Requirements:
A pre-assignment medical evaluation will need to be accomplished no more than thirty (30) calendar days prior to submission of credentials. The purpose of this medical evaluation is to determine if the individual, from a medical standpoint, possesses the minimum physical abilities needed to perform the proposed job without significant risk to personal health or the health and safety of others. As a minimum, the individual shall be free of physical defects or medical conditions which might reasonably be expected to place other workers, patients, or the public at risk.
